Yes, I know to practice. I currently average around 40-45s. I average 17-18s on regular solving using cfop. Should I... improve cross, PLLs, what first?

Well, nobody can really tell what your problem is just based on your time. (Take a video?) But I can tell you some general stuff: practice doing 2gen solves/algs as fast as possible, try to look ahead more than on your normal solves, try to limit yourself to R/U/r/u turns as much as possible, make your cube even looser than for two-handed solves.

Haha, I was about to make a video about that. My tips:

  • Make sure you can turn / hold the cube properly and do reasonable TPS
  • Have no pauses. Try doing a solve with super slow turning 2H. That should be your goal time for OH. Super fast cubers like Michal Pleschoss (bleh) don't turn super fast (maybe it's just me), but are efficient and barely stop turning.
  • Learn better OH algs. I've got OH PLL, 2LOLL and full OLL on my site.
  • Use a cube you're comfortable with. Some people prefer different sized cube, different speeds, but just make sure you're comfortable and able to use it properly.
  • Practice slow solves and get your technique down.

I hope this was helpful!
